Let us kindly express our appreciation of your interest in employment in our company, and at the same time ask you to fill in all required data necessary for us to be able to contact you.

By sending this application form I hereby grant my permission to process all given personal data in the information system of the potential employer. Personal data are processed to consider and register the job application form, as well as the conclusion of labor-management relation. If the employment contract is concluded, the personal data will be processed for the wages and tax purposes, as well. The permission is granted for the period of six months from the date of filling-in/delivery of this application form to the potential employer, and in case of the employment contract conclusion it is valid during the whole period of employment and also six months after its termination. This permission can be cancelled anytime and without stating the reason until the conclusion of employment contract. Job applicant also hereby declares that the given personal data are correct and he/she is authorized to provide such data to the potential employer.
